Since 2019, Creative Pathways has been committed to bringing support, hope and healing to those facing critical life challenges. Our methods have developed over the years, and with more resources and more support from an increasingly aware community, we are able to do more for our victims each day.
Creative Pathways focuses on supporting victims immediately following a crisis or traumatic experience through their long-term recovery. As a for-profit organization, we are committed to providing the highest quality services. Our main goal is to change lives by bringing relief to those who have been through a traumatic experience. Everyone involved with the services of Creative Pathways is empathetic, highly trained and qualified in providing victims of abuse and trauma with the help they need to move ahead. Sliding Fee Discount Program
Creative Pathways, Inc. believes that no one should be denied access to behavioral health care because of an inability to pay.
In keeping with this commitment, Creative Pathways, Inc. offers a Sliding Fee Discount Program (SFDP) to all patients who qualify. The program is available to uninsured and underinsured individuals and families and is designed to make integrated mental health and substance use disorder services affordable regardless of financial circumstances.
Who is eligible
Eligibility is based on family size and annual household income, measured against the most current U.S. Department of Health and Human Services Federal Poverty Guidelines (FPG). The program is available to all patients of Creative Pathways, Inc., regardless of insurance status, who meet the income and family size criteria.
- Patients with household incomes at or below 100% of the FPG receive a full discount and are not charged any fee for services. Creative Pathways, Inc. does not charge a nominal fee.
- Patients with household incomes above 100% and at or below 200% of the FPG receive a partial discount on a sliding scale, as shown below.
No one is denied services based on inability to pay. Patients who are unable to pay the full amount owed after discounts are applied are encouraged to speak with our Billing Office to discuss payment arrangements.
How To Apply

Applications are reviewed promptly, and patients are notified of the discount tier for which they qualify. Eligibility is reviewed annually or sooner if a patient’s circumstances change.

The sliding fee schedule is updated annually based on the most current Federal Poverty Guidelines.
